If you are a former client of MCC and need records, you can request them from us. If the party you want to share records with does not have their own form, or if you are requesting information for yourself, please use the request form below.

By law mental health records are kept for 6 years from the last date of service for adults. For minor clients, records are kept for 7 years past the age of majority.
